QUICK AND EASY WAYS TO ENROLL WITH CMA:

  • Contact our office to schedule an appointment
    • We will train you on the food program requirements.
    • We will teach you how to use the software.
    • We will help you complete all the required paperwork.
  • Have one of the following Child Care License available:
    • A current Utah Family Child Care License or Residential Certificate.
    • A current Child Care Center License.
    • Relative Care Certificate (approved through CMA).
    • Alternate Care Certificate (approved through CMA).
    • Family Friend & Neighbor Approval (DWS FFN)
